Abstract

The exposome refers to an emergent research field, the aim of which is to develop an integrated science of
the totality of exposures to which an individual is subjected over the course of their life and which influence
their health. There are two principal research orientations. One concentrates on the study of the internal
exposome, which makes it possible to identify the main exposures impacting on health by precisely measuring
them at the level of the biochemical environment of the body. The other aims to integrate social and
ecological exposures with biological and internal exposures and thus to develop a holistic model of environmental
health. Taking as its point of departure the examination of the scientific literature, the analysis
of the first generation of exposomic studies (2010-2020), and interviews conducted with epidemiologists
engaged in this research, this article offers a review of the promises and realizations of this emerging field
as regards the integration of exposures, while also seeking to offer a critical perspective on this topic. After
having presented the exposome and its diverse conceptions, I consider which type of integration is in play in
each of these two orientations, while also considering their interest and pertinence. The analysis concentrates
above all on the second orientation: what does this highly holistic approach have to offer as regards
the renewed integration of environmental and genetic factors, but also social and biological factors, in our
understanding of exposures and our explanation of disease?
